Care For You at Home has been supporting the community of Port Stephens and beyond with high-quality care services in the areas of Community Nursing, Disability, and Home Care since 1991. Their team offers a range of personalised services to meet every individual’s needs – from domestic assistance to home maintenance and registered nursing such as palliative care or wound dressing management.

In early 2022 Care For You at Home was acquired by MS Plus who is the go-to provider of information, advice, and support for people affected by multiple sclerosis and other neurological conditions.

About the opportunity

The Clinical Manager is responsible for clinical oversight of care and support services delivered to the clients of Care For You at Home including services brokered to other providers. They will ensure effective and efficient person-centered care of all clients and that delivery of care is tailored to meet the individual needs and preferences of the clients.

Key duties:

  • Manage the delivery of clinical care that is evidence-based, safe, effective, person-centered and that meets changing clinical needs

  • Leadership and management of Care For You at Home’s RN’s, Allied Health staff and Community Care Workers with a focus on training, development, and education

  • Provide quality reporting to the Executive and Board of MS Plus, including the understanding and compliance to relevant regulatory standards, clinical safety and quality indicator data, and client feedback

  • Implementation of the Clinical Governance Framework to drive safety and quality

  • A system for the safe and quality use of medicines is implemented and related processes are monitored for adherence by workers and updated as required

About you

The successful candidate will have 5 years of experience as a Registered Nurse with a current AHPRA registration. Experience working within aged care is required and it would be highly desirable to have worked in a similar in-home aged care service. We are looking for candidates that understand the Aged Care Quality Standards and Aged Care Legislation with a sound understanding of how to apply these principles to home-based care delivery.

In addition, we are looking for:

  • Strong interpersonal skills with the ability to build relationships with all stakeholders, including clients, families/carers, and staff

  • A passion for working within the aged care sector and improving the lives of the clients by putting them at the centre of all decision-making

  • Excellent report writing and analysis skills, reporting is delivered timely, accurately, and professionally

  • Highly organised with excellent attention to detail and administrative skills

  • Proven experience in proactively identifying risk and areas of improvement and acting on this to improve the quality of care and meet regulatory requirements

  • Experience developing engaging and effective training and education for the team is desirable
