About the role
Due to growth, we are looking for 3 x Aged Care Service Coordinators who will be responsible for the coordination and delivery of the Commonwealth Home Support Program services to persons 65 years of age of over from predominantly Culturally and Linguistically Diverse (CALD) backgrounds. Service Coordinators follow a strength based, person centered approach with a focus on encouraging independence and empowerment for their clients.

Key duties include:

  • Provide leadership and management for your team of care support workers to deliver care, assistance, and support efficiently and effectively to clients across multiple locations.

  • Provide a holistic, strengths-based, person-centred assessment, including any clinical screening, of each client’s needs, strengths, abilities, and interests.

  • Consult with Mosaic care planners, clients, and their circle of support to develop a care plan that facilitates the individual’s goals.

  • Implement the care plan to provide coordinated assistance, support, and activities for clients.

  • Escalate client complex needs to Mosaic care partners for comprehensive case management and response.

  • Regularly monitor and review care needs, identify gaps in service delivery and supports, and amend care plan and service delivery as required.

About you
We are looking for engaged and innovative individuals who are passionate about delivering the best outcomes for their clients. The successful candidate will demonstrate a growth mindset with a focus on continuous improvement, along with:

  • Relevant experience in case managed community aged care or community services paired with demonstrated success of engaging effectively with aged persons, family members, carers, staff, and other service providers.

  • Experience providing leadership and coordination to a team including rostering and allocating resources efficiently and effectively.

  • Demonstrated cultural competence and ability to adapt to support the client effectively. Lived experience and/or experience working with people from CALD backgrounds will be highly regarded.

  • High level of computer proficiency including experience utilising a CRM (Sanwai desirable) and Microsoft Office programs.

  • Minimum Certificate IV level of education in relevant discipline, for example ageing support, community services, or administration.
