Brightwell Health is seeking an experienced and compassionate Nurse Practitioner to join their multidisciplinary outreach team. This permanent part-time position offers the opportunity to deliver person-centred, trauma-informed care in the community - bringing healthcare directly to women and families who have experienced domestic violence and homelessness living in crisis accommodation across the Hunter and Lake Macquarie regions. This is an opportunity to join a groundbreaking multiagency outreach health program in partnership with the Primary Health Network.
What you’ll do
Deliver autonomous, evidence-based care including assessment, prescribing, referrals and health management
Provide trauma-informed, developmentally appropriate care for women and children
Collaborate with GPs, allied health and community partners to ensure wraparound, integrated care
Promote health literacy, prevention, and empowerment through education and advocacy
Lead and contribute to quality improvement and clinical governance initiatives
About you
Current AHPRA registration with endorsement as a Nurse Practitioner
Strong clinical judgement and advanced assessment skills
Excellent communication, leadership and collaboration abilities
NSW Working with Children Check, CPR certification and unrestricted driver’s licence
A values-driven approach grounded in compassion, fairness and respect
Desirable
Experience in primary or community care, women’s health, or trauma-informed practice
Knowledge of RACGP standards and sexual/reproductive health
